A seizure is a transient event caused by abnormal brain electrical activity. Epilepsy is a predisposition to recurrent unprovoked seizures.

Possible features

Loss of awareness, convulsive movements, staring/behavioural arrest or episodic confusion may occur.

Assessment

A detailed history, eyewitness/video account, examination and appropriate tests such as ECG, EEG or imaging help determine the diagnosis. NICE recommends urgent specialist assessment after a first suspected seizure.

Treatment

Treatment depends on seizure type, syndrome, recurrence risk and patient circumstances.

Emergency

A prolonged convulsive seizure or repeated seizures without recovery is an emergency.

Frequently asked questions

Does a normal EEG exclude epilepsy?
No. EEG supports diagnosis in the appropriate context but is not used to exclude epilepsy when normal.
What should I bring?
A witness account or safe video of the event can be very helpful.
